Default SEO: How Do You Write A Keyword Research Report?

I'd like to do put together a keyword research for each of my 4 pages: 1. home, 2.about us, 3.services, 4.categories and 5.agreement.

How do I go about putting a report together? What should be included? Any examples online of a well written keyword research report?
